If you know the following details, please enter them. It will make it easier to find your driver record.
The application process requires the DVLA to check your identity and the details you have given us with third parties. By providing the details below you are giving permission for your details to be submitted to these third parties via the DVLA. If we can't confirm your identity with the DVLA, we will notify you via email.
Please note if you don't enter your NI number below it may result in you having to send the DVLA additional paper documentation such as original identity documents by post.
By entering your National Insurance number you will provide DVLA with consent to check your personal details and National Insurance number with the Department of Work and Pensions (DWP) and Her Majesty's Revenue and Customs (HMRC).
Please enter your valid UK passport number below. You will be giving permission for the DVLA to access your personal data (including your photo and signature) held by Her Majesty's Passport Office (HM Passport Office).
The DVLA will use this information to verify your identity. This is not required although if you do not provide this information you be required to complete a D1 form

I understand that it is a criminal offence to make a false declaration or withhold material information in order to obtain a driving licence and that to do so can lead to prosecution and a fine of up to £1,000 or imprisonment for a period of up to two years or both
I understand that it is a legal requirement that only one driving licence is held at one time and confirm that I have cut my driving licence in two and will return it to DVLA.
